What goes on a title page?
APA 7 (student): paper title in bold, your name, department and university, course number and name, instructor, and due date, centered in the upper half of the page.
MLA: no title page. Put a four-line heading at the top left of page one, then the title centered.
UK and Australian cover sheets: usually include your student number, module or unit code, word count and a declaration of originality. Many universities supply their own template, so use it if one exists.

No. Student papers only need a page number in the top-right corner unless your instructor asks for a running head.
